To reserve a seat in the cohort we will need your application, registration, and $60 application fee (Includes 1-year OAESA Associate Membership). These items are found in Steps 1-3 and can be completed by clicking the button(s) next to each step.

Financial Aid & Payment Options

If using Financial Aid, Complete the FAFSA online. It is in your best interest to complete the financial aid process as early as possible.

Letter(s) of Recommendation

The easiest and fastest way to request a recommendation is to use our online system by clicking the button. This will allow us to e-mail reference forms directly to your recommender on your behalf. Letters should be from individuals able to comment on your academic potential, personal character, and competence and effectiveness in professional work.


Official Transcripts

Transcripts must be sent directly to CUC from the college or university where you received your degree(s). Arrange for official, sealed transcripts to be sent to:

Objective Statement

This statement should include information on your future educational goals and/or objectives and their relationship to pursuing graduate work at Concordia. Please upload your completed essay by clicking the button.


Teaching License

Ohio licensure students only:


Employment Verification Form

Please submit the required Employment Verification form to be completed by your employment supervisor or applicable school/district human resources employee. Your employment verification must indicate two full years of teaching experience for the Ohio Principal Licensure programs and the Ohio Teacher Leader programs. Two years of successful administrative experience is required prior to enrolling in a program leading to Ohio Superintendent Licensure.
